Jr & Sr High Boating Day 2025

Jr & Sr High, come join us on Sunday July 27th from 2-6pm for a boating day at the Venzara home! We will be tubing, swimming, and hanging out together! Dinner will be provided. Please bring a towel, sunscreen, and a change of clothes. Please wear a modest swim suit. There is no cost but online registration is required. I hereby grant permission for my child included on this form to attend and participate in the Jr & Sr High Boating Day on Sunday July 27th, 2025 at The Venzara's Home and agree to the following guidelines:

1. I hereby give my permission for medical attention to be given to my student in the event of injury, illness or accident, including major surgery. I realize that I will be contacted at the earliest possible time in case of accident.

2. I hereby release Calvary Chapel Merritt Island, and the representatives of the church from liability in case of accident.

3. I hereby give permission to Pastor Josh and Youth Leaders to call their guardian to pick up their child in case of inappropriate behavior and/or discipline issues.

4. An inherent risk of exposure to COVID-19 exists in any public place where people are present. I voluntarily assume all risks related to exposure to the COVID-19 virus. And here by release Calvary Chapel Merritt Island and the representatives of the church from liability due to contracting the COVID-19 virus
